Pathway Reimbursement Support

Abbott Nutrition believes that access to products recommended by healthcare providers is important for all patients. If you do not have access to alternative sources of coverage or funding of Abbott Nutrition products, you may qualify for Abbott Nutrition’s Patient Assistance Program. This program is designed to provide supplemental product at no cost to eligible patients experiencing financial difficulties.

AbbVie Duopa Patient Assistance Program

AbbVie sponsors a program called DuoConnect which offers many patient support services including financial assistance. AbbVie also offers a patient assistance program which provides Duopa at no cost to eligible patients who do not have insurance coverage for Duopa and do not have access to other funding sources.

The AbbVie Patient Assistance Foundation provides AbbVie medication at no cost to people experiencing financial difficulty who may or may not have or qualify for prescription medication benefits through private insurance or government-funded programs.

AbbVie Patient Assistance Foundation (Lupron Depot and Lupaneta Pack)

The AbbVie Patient Assistance Foundation provides free Lupron Depot® (leuprolide acetate for depot suspension) and LUPANETA PACK (leuprolide acetate for depot suspension and norethindrone acetate tablets) to patients experiencing financial difficulties.

AbbVie Patient Assistance Foundation (Moderiba)

The AbbVie Patient Assistance Foundation provides Moderiba (ribavirin) at no cost to patients experiencing financial difficulties.

Axona Reimbursement Program

Accera is committed to helping people with Alzheimer's disease and their caregivers, regardless of their financial situation. By calling the Axona Reimbursement Helpline a Reimbursement Specialist will assist you with your reimbursement-related inquiries.

RAPAFLO^® Patient Assistance and Reimbursement Programs

The RAPAFLO^® Patient Assistance Program (PAP) helps uninsured patients gain access to RAPAFLO^®. Eligibility for PAP is determined based on various medical and financial qualifications. Zavesca Patient Support

Zavesca Patient Support is a financial assistance program available to eligible patients who have been prescribed Zavesca. This program may cover your out-of-pocket  and other costs while you are taking Zavesca, so there is no cost to you.

Akrimax Patient Assistance Program

The VIVITROL® Value Program was created to help you with treatment costs.

BOTOX PATIENT ASSISTANCE Program

The program provides BOTOX® at no charge to financially eligible patients. Those who may qualify include patients who are uninsured or underinsured. Allergan is proud to assist eligible uninsured and underinsured patients with their treatment through the donation of BOTOX® vials.

Patient Assistance Program RxHope

The Patient Assistance Program provides assistance to uninsured patients that cannot afford their medications. Annual household income limits do apply but each case is reviewed on an individual basis. Patients must reside in the U.S. and be under the care of a U.S. based physician.

The Arizona Department of Health Services (ADHS) is responsible for the administration of ADAP for the state of Arizona. ADAP provides access to medications used to treat HIV and prevent the onset of related opportunistic infections to low-income individuals with HIV disease who have limited or no coverage from private insurance or Medicaid.

Arkansas AIDS Drug Assistance Program

AIDS Drug Assistance Programs (ADAPs) are state administered programs that provide HIV/AIDS medications to low-income individuals living with HIV disease who have little or no coverage from private or third party insurance.

AZ&Me™ Prescription Savings Program for people with Medicare Part D

The AZ&Me™ Prescription Savings program for people with Medicare Part D is designed to provide AstraZeneca medicines at no cost to qualified patients enrolled in a Medicare Part D prescription drug coverage plan but who are having difficulty affording their AstraZeneca medicine(s).

AZ&Me™ Prescription Savings Program for people without insurance

The AZ&Me™ Prescription Savings program for people without insurance is designed to provide AstraZeneca medicines at no cost to qualified patients. This patient prescription assistance program can help patients who do not have prescription drug coverage and who meet other eligibility criteria.

Nuedexta Patient Assistance Program

A six month supply is provided to those who are eligible. Anyone requesting assistance can call (855) 468-3339 to request a faxed application or download it from the website. Health care providers and their staff members, and Patients/Caregivers can initiate the application process online by visiting the company website.

Bausch & Lomb Patient Assistance Program

Donnatal Patient Assistance Program

In order to ensure that all of our customers have access to the medication that their doctors think is most beneficial the Donnatal team has created the Donnatal Patient Assistance Program to offer financial assistance to customers that may otherwise be unable to afford Donnatal®.

Connecticut AIDS Drug Assistance Program

This program pays for drugs determined by the U.S. Food and Drug Administration to prolong the life of people with AIDS, or HIV infection. To be eligible for the program in Connecticut, an individual must have a physician certification that the individual has HIV infection, HIV disease or AIDS, must not be a recipient of Medicaid, and must have net countable income within 400% of the Federal Poverty Level. In addition, the individual must apply for Medicaid within two weeks of approval for this program.

The department receives federal funding for the program under Ryan White Title II grants that are awarded to the Department of Public Health.

Korlym Pateint Assistance Program

If another supply is needed, the physician is to submit another enrollment form.*

Duchesnay USA Patient Assistance Program

The Florida AIDS Drug Assistance Program provides AIDS-specific medications at no cost to residents who qualify.

Forest Pharmaceuticals Patient Assistance Program

This program provides certain Forest products to patients who are unable to afford the cost of their medication and who meet other eligibility requirements. Eligibility for this program is based upon information you and your licensed practitioner provide on the application form. If you are approved, you will receive a three-month supply of the Forest product you require at no charge. Forest will ship your medication to your licensed practitioner's office for them to dispense to you.

Phoslyra Patient Assistance Program

Phoslyra Patient Assistance Program provides Phoslyra to patients with no drug coverage or patients who have Medicare Part D only.

Venofer Patient Assistance Program (free-standing dialysis centers only)

Genentech® Transplant Access Services connects patients to the Valcyte medicine they need.

The Charitable Access Program (CAP Program)

Because many of the conditions that Genzyme's therapies treat are rare, our patients often face challenges with insurers that are unfamiliar with these diseases and their unique therapies. Many people encounter restrictions in their coverage – or have no medical coverage at all. The Charitable Access Program provides such assistance.

Genzyme Charitable Access Program (CAP)

Genzyme's Charitable Access Program (CAP) was established in the United States in order to provide Genzyme's enzyme replacement therapies free of charge to qualified U.S. patients whose financial circumstances prevent their access to treatment. The program is committed to providing Lumizyme to patients who: Have a demonstrated medical need for treatment, are uninsured or have inadequate insurance coverage.

Renal Patient Assistance Program

The Renal Patient Assistance Program is designed for patients in financial need who do not have coverage for Renvela® (sevelamer carbonate) (and/or Hectorol® [doxercalciferol injection]) under any prescription drug benefit, including commercial insurance, Medicare, Medicaid, or other government insurance programs.

Renvela^® Medicare Part D Assistance Program

PDAP provides a free supply of Renvela® (sevelamer carbonate) to eligible patients with Medicare Part D who cannot afford their co-payments or co-insurance for their prescriptions.

Thyrogen Patient Assistance Program (PAP)

Genzyme assistance program that provides Thyrogen at no cost to eligible patients who do not have insurance or whose insurance does not cover Thyrogen.

Georgia AIDS Drug Assistance Program

AIDS Drug Assistance Programs (ADAPs) are state administered programs that provide HIV/AIDS medications to low-income individuals living with HIV disease who have little or no coverage from private or third party insurance. ADAP must be the payer of last resort.

Gilead U.S. Advancing Access^®

This program provides assistance to patients in the United States who do not have insurance or who need financial assistance. As part of this program, Gilead provides assistance for people who are eligible and who cannot afford to pay for TRUVADA.

Support Path for Sovaldi^®

Support Path™ assists eligible hepatitis C patients in the United States who do not have insurance, are underinsured or who otherwise need financial assistance to gain coverage for or access to Sovaldi® (sofosbuvir) or Harvoni® (ledipasvir/sofosbuvir).

U.S. Advancing Access^®

Gilead’s U.S. Advancing Access® program provides assistance to patients in the United States who do not have insurance or who need financial assistance.

Zydelig Patient Assistance Program

If you lack insurance coverage or have been denied coverage, ZYDELIG® AccessConnect™ may be able to help. If you have no insurance coverage The Patient Assistance Program can determine if your financial situation qualifies you to receive your medication, free of charge. If your insurance claim has been denied, one of the program specialists can help identify the reason for the denial, contact your insurer to determine the appeal process, and provide this information to your doctor.

BENLYSTA Patient Assistance Program

BENLYSTA Patient Assistance Program provides free product to eligible, uninsured patients.

Eli Lilly and Company offers a free program called the Humatrope DirectConnect (HDC). The HDC is designed to help guide you through the drug authorization process required by your insurer for Humatrope® therapy. The HDC will investigate your insurance coverage to determine if you have coverage for Humatrope and inform you of your options. The HDC will help you through this process and answer questions you have.

If it is determined that your insurer will not cover your Humatrope therapy and you meet certain criteria, the HDC will provide you with paperwork to apply for Humatrope ACCESS. If you qualify for the program, Humatrope ACCESS will provide free Humatrope medicine to you via mail order pharmacy.

Forteo Patient Assistance

The FORTEO Patient Assistance program is for non-Medicare eligible individuals. If you qualify for the program, FORTEO Patient Assistance Program will provide free FORTEO medicine to you via mail order pharmacy. Prescription refills will be available during your 1-year enrollment period.

Lilly Cares

If you qualify for the Lilly Cares program, a 120-day supply of medicine will be shipped to your health care provider's office for you to pick up. Prescription refills will be available during your 1-year enrollment period. For refills, please complete and submit the fax refill form.

LillyMedicareAnswers Patient Assistance Program

If you qualify for the program, LillyMedicareAnswers will provide free Lilly medicines to you through a mail order pharmacy.

PatientOne Lilly Oncology Patient Assistance Program

If you qualify for the program, PatientOne will provide free oncology medicine to you shipped to your physician. Approved product requests are conveniently shipped to your doctor's office. The PatientOne program may provide product assistance to qualified applicants for up to three months pending other coverage such as Medicaid, Medicare, or Department of Veterans Affairs (VA) health care coverage.

Louisiana AIDS Drug Assistance Program

The Louisiana Office of Public Health STD/HIV Program has been the primary public entity responsible for administering, implementing, and monitoring HIV prevention interventions for the State of Louisiana since 1985. These prevention services have from the beginning included HIV counseling and testing, outreach and condom availability.

Montana AIDS Drug Assistance Program

The Montana STD/HIV Section administers the AIDS Drug Assistance Program (also known as ADAP) with funding provided by the Ryan White Part B CARE Act which is administered through the federal Health Resources and Services Administration. This program provides HIV anti-retroviral drugs, medications to prevent opportunistic infection, and certain drugs to treat HIV-related disease for individuals who are uninsured or under-insured and who are unable to pay for such treatment.

NORD Patient Assistance Program

Since 1987, NORD has administered Patient Assistance Programs to help patients obtain life-saving or life-sustaining medication they could not otherwise afford. Over the course of time NORD has expanded its services to offer financial assistance with insurance premiums and other patient fees as well as assistance with reasonable and appropriate diagnostic testing expenses and travel to and consultation with disease specialists that are not covered by a patient’s insurance plan.

Sometimes insurance coverage takes time to kick in. This can happen because of normal processing time, but coverage can also be delayed if your initial claim is denied and your NordiCare® Case Manager is working through the appeals process. If there will be a delay in getting insurance coverage, your Case Manager may talk with you about the JumpStart™ program, through which you can receive free Norditropin® (somatropin [rDNA origin] injection) as long as your insurance claim is actively in process. You will never have to repay NordiCare® for this medication, even if your claim is eventually denied. Even after your insurance coverage begins, there may be times that JumpStart™ can help out. Your Case Manager will step in again, and get you going with JumpStart™ if necessary, if there’s a risk of interrupting therapy caused by any of these circumstances:

SevenASSIST

SevenASSIST can aid in relieving the financial burden by offering people with congenital hemophilia with inhibitors, congenital factor VII deficiency, or acquired hemophilia a premium support service, available for free in a convenient Web-based portal. Patients who meet any of these criteria may qualify for product assistance for up to 6 months or up to a quantity of 300 mg, whichever is greater, at no cost.

NPS Advantage Support Program

NPS Advantage is a support program, sponsored by NPS Pharma, that provides assistance to patients and health care professionals at no charge.

Oklahoma AIDS Drug Assistance Program

The HIV Drug Assistance Program (HDAP), through the Oklahoma State Department of Health and the contracted pharmacy, OU Pharmacist Care Center, provides specific HIV medications to eligible low-income individuals living with HIV disease in
Oklahoma (through mail order or pharmacy). HDAP is funded by the Federal Ryan White CARE Act Part B grant to the states. HDAP is the payor of last resort.

Zubsolv Patient Assistance Program

This program provides 1,000 eligible patients with monthly supplies of ZUBSOLV as prescribed by their physician at no charge. Patients must demonstrate continued eligibility every six months. The people who work at Vertex understand that medicines can only help patients who can get them. With that in mind, Vertex provides a comprehensive financial assistance and patient support program for its approved medicine - KALYDECO™ (ivacaftor). The program, known as Vertex Guidance and Patient Support (GPS), helps people learn about insurance benefits, gives Vertex medicines for free to eligible patients who do not have insurance and provides coverage for co-pay or co-insurance costs associated with KALYDECO for people who meet certain program criteria.

Pre-Exposure Prophylaxis Drug Assistance Program (PrEP DAP)

West Virginia AIDS Drug Assistance Program

AIDS Drug Assistance Programs (ADAPs) are state administered programs that provide HIV/AIDS medications to low-income individuals living with HIV disease who have little or no coverage from private or third party insurance. ADAPs were originally authorized by the Ryan White Comprehensive AIDS Resources Emergency (CARE) Act, which was enacted in 1990, and reauthorized in 1996 and again in 2000. Currently, ADAPs are authorized under the Ryan White HIV/ADIS Treatment Modernization Act of 2006. The intent of state and federal legislation is to assure that ADAP funds are used only for the purchase of ADAP formulary drugs that cannot be paid for through other sources. ADAP must be the payor of last resort.

Wisconsin AIDS/HIV Drug Assistance Program (ADAP)

The Wisconsin AIDS/HIV Drug Assistance Program (ADAP), authorized by s. 49.686 of the Wisconsin statutes, is designed to maintain the health and independence of persons living with HIV infection in Wisconsin by providing access to HIV-related antiretroviral drugs and prophylactic medications as well as vaccines for hepatitis A and B. The ADAP formulary also includes hepatitis C virus (HCV) medications for persons co-infected with HIV and HCV.

Wyoming AIDS Drug Assistance Program (ADAP)

The Wyoming Department of Health's HIV/AIDS Treatment and ADAP Program provides persons living with HIV/AIDS access to comprehensive health care and other supportive services. These programs serve as the payer of last resort for persons who are uninsured, under-insured, or are otherwise unable to access medical treatment. Funding for these programs is provided, in part, by grants from the United States Department of Health and Human Services, Health Resources Services Administration, and the United States Department of Housing and Urban Development.

Xubex Patient Assistance Program

Xubex Patient Assistance Program (PAP) is a program which helps patients who can't afford their high cost medications. It helps qualifying patients who do not have prescription drug coverage to get free or low-cost medicines through the public or private programs that's suited for them.
